Housing

Transitions of PA has housing programs that are referral-based programs only, from the PA 2-1-1, HMIS, Coordinated Entry System. 

 

Individuals and families seeking services administered by Transitions of PA, must engage, and have an assessment completed, with 2-1-1 prior to entry into one of the housing programs, to ensure they meet the criteria. Call 2-1-1 to speak to a housing specialist or text zip code to 898-211.

Process of referrals and Housing program entry:

 

Once Transitions of PA, receives a referral from the Coordinated Entry Regional Manager, from 2-1-1; a survivor that is fleeing a domestic violence situation that puts them at risk of homelessness, or in a homelessness situation, Transitions of PA can offer them assistance in one of our Housing Programs.  Any survivor in the housing program will also be  provided with continued support, life skills, budgeting assistance, case management services and outside resources, to empower the survivors that Transitions of PA, Housing Advocates are engaging with, obtain and maintain housing while working towards goals and self-sufficiency.
